A notification of the commencement of guardianship that will commence in the case prescribed in Article 838, item (i) of the Civil Code (referred to below as "guardianship over a minor") must be submitted by the guardian of a minor designated pursuant to the provisions of Article 839 of the Civil Code within ten days from the day on which they assumed the role of guardian.
民法第八百三十八条第一号に規定する場合に開始する後見（以下「未成年者の後見」という。）の開始の届出は、同法第八百三十九条の規定による指定をされた未成年後見人が、その就職の日から十日以内に、これをしなければならない。
The written notification must state the following particulars and be accompanied by a copy of the will relating to the designation of the guardian of a minor:
届書には、次に掲げる事項を記載し、未成年後見人の指定に関する遺言の謄本を添付しなければならない。
the cause and date of the commencement of guardianship; and
後見開始の原因及び年月日
the date that the guardian of the minor assumed that role.
