A request for examination under the preceding Article must be filed by submitting a written request for examination, as provided by Ministry of Justice Order.
前条の規定による審査の申立ては、法務省令で定めるところにより、審査申立書を提出してしなければならない。
The following matters must be stated in the written request for examination referred to in the preceding paragraph:
前項の審査申立書には、次に掲げる事項を記載しなければならない。
the content of the disposition, etc. to which the request for examination relates;
審査の申立てに係る処分等の内容
the purport of and reasons for the request for examination; and
審査の申立ての趣旨及び理由
other particulars provided for by Ministry of Justice Order.
