A person who intends to obtain registration referred to in the preceding Article must, pursuant to the provisions of Cabinet Office Order, submit a written application for registration containing the following particulars to the Prime Minister:
前条の登録を受けようとする者は、内閣府令で定めるところにより、次に掲げる事項を記載した登録申請書を内閣総理大臣に提出しなければならない。
trade name or other name, and address;
商号又は名称及び住所
the amount of capital or contribution;
資本金又は出資の額
name and location of the business office or office pertaining to the business of issuing prepaid payment instruments;
前払式支払手段の発行の業務に係る営業所又は事務所の名称及び所在地
names of officers;
役員の氏名又は名称
the type, the name, and the amount available for payment, etc. of the prepaid payment instruments;
if a specified period of time which is available for the purpose of paying consideration for the purchase or renting of goods, etc. or the receipt of the service being provided or for the purpose of claiming the delivery or provision of those goods, etc. or services, or the expiry date is set for the prepaid payment instruments, the period of time or due date;
物品等の購入若しくは借受けを行い、若しくは役務の提供を受ける場合にこれらの代価の弁済のために使用し、又は物品等の給付若しくは役務の提供を請求することができる期間又は期限が設けられているときは、当該期間又は期限
the details and means of the business of issuing prepaid payment instruments;
前払式支払手段の発行の業務の内容及び方法
if a part of the business of issuing prepaid payment instruments is entrusted to a third party, the details of the business pertaining to the entrustment, the name or trade name or other name and address of the third party to whom the business of issuing prepaid payment instruments is entrusted;
前払式支払手段の発行の業務の一部を第三者に委託する場合にあっては、当該委託に係る業務の内容並びにその委託先の氏名又は商号若しくは名称及び住所
location and contact address of the business office or office that handles complaints from the users about the issuance and use of prepaid payment instruments or provide consultations for the users; and
前払式支払手段の発行及び利用に関する利用者からの苦情又は相談に応ずる営業所又は事務所の所在地及び連絡先
other particulars specified by Cabinet Office Order.
その他内閣府令で定める事項
A document in which the applicant pledges that the applicant does not fall under any of the items of Article 10, paragraph (1), documents concerning finance, and other documents specified by Cabinet Office Order must be attached to the application for registration referred to in the preceding paragraph.
