The administrator of a hospital, etc. subject to a report on medical institution functions, etc. that provides outpatient medical care (hereinafter referred to as a "hospital, etc. subject to a report on outpatient functions" in this Article) must, in order to promote the differentiation and coordination of the functions of hospitals and clinics relating to outpatient medical care in the region, report the following particulars to the prefectural governor of the location of the hospital, etc. subject to a report on outpatient functions, pursuant to an Order of the Ministry of Health, Labour and Welfare:
医療機関機能等報告対象病院等であつて外来医療を提供するもの（以下この条において「外来機能報告対象病院等」という。）の管理者は、地域における外来医療に係る病院及び診療所の機能の分化及び連携の推進のため、厚生労働省令で定めるところにより、次に掲げる事項を当該外来機能報告対象病院等の所在地の都道府県知事に報告しなければならない。

in the case where the hospital, etc. subject to a report on outpatient functions intends to play, in the region, the role of a core hospital or clinic that provides the outpatient medical care specified by an Order of the Ministry of Health, Labour and Welfare under the preceding item, that fact; and
当該外来機能報告対象病院等が地域において前号の厚生労働省令で定める外来医療を提供する基幹的な病院又は診療所としての役割を担う意向を有する場合は、その旨

その他厚生労働省令で定める事項
If the administrator of a hospital, etc. subject to a report on outpatient functions fails to make a report pursuant to the provisions of paragraph (1) (excluding the part pertaining to item (ii)) or has made a false report, the prefectural governor may order the establisher of the relevant hospital, etc. subject to a report on outpatient functions to have the relevant administrator give the relevant report or correct the details of the relevant report within a period that the governor prescribes.
都道府県知事は、外来機能報告対象病院等の管理者が前項（第二号に係る部分を除く。）の規定による報告をせず、又は虚偽の報告をしたときは、期間を定めて、当該外来機能報告対象病院等の開設者に対し、当該管理者をしてその報告を行わせ、又はその報告の内容を是正させることを命ずることができる。
The provisions of Article 30-13, paragraphs (3), (4) and (6) apply mutatis mutandis to a report under the provisions of paragraph (1). In this case, the term "hospital, etc. subject to a report on medical institution functions, etc." in paragraph (3) of that Article is replaced with "hospital, etc. subject to a report on outpatient functions", and the term "preceding paragraph" in paragraph (6) of that Article is replaced with "Article 30-18-2, paragraph (2)", and "hospital, etc. subject to a report on medical institution functions, etc." with "hospital, etc. subject to a report on outpatient functions".
第三十条の十三第三項、第四項及び第六項の規定は、第一項の規定による報告について準用する。この場合において、同条第三項中「医療機関機能等報告対象病院等」とあるのは「外来機能報告対象病院等」と、同条第六項中「前項」とあるのは「第三十条の十八の二第二項」と、「医療機関機能等報告対象病院等」とあるのは「外来機能報告対象病院等」と読み替えるものとする。
