Article 16Notifications, etc. under Article 17, paragraph (1) of the Supplementary Provisions of the amending Act
第十六条（改正法附則第十七条第一項の届出等）
A notification under Article 17, paragraph (1) of the Supplementary Provisions of the amending Act (excluding a notification deemed under paragraph (3) of that Article to be a notification under paragraph (1) of that Article) or a notification under Article 18, paragraph (1) of the Supplementary Provisions of the amending Act (excluding a notification deemed under paragraph (3) of that Article to be a notification under paragraph (1) of that Article) must be made by submitting one written notification in the format of Appended Form No. 5.
改正法附則第十七条第一項の規定による届出（同条第三項の規定により同条第一項の規定による届出とみなされる届出を除く。）又は改正法附則第十八条第一項の規定による届出（同条第三項の規定により同条第一項の規定による届出とみなされる届出を除く。）は、別記第五号様式による届出書一通を提出して行わなければならない。
