A person who seeks to revert to their previous surname pursuant to the provisions of Article 791, paragraph (4) of the Civil Code must state this and the date of reversion to their surname under paragraphs (1) thorough (3) of the relevant Article in the written notification, and submit it.
民法第七百九十一条第四項の規定によつて従前の氏に復しようとする者は、同条第一項から第三項までの規定によつて氏を改めた年月日を届書に記載して、その旨を届け出なければならない。
If the person referred to in the preceding paragraph has a spouse, they must submit the notification jointly with their spouse.
