A notification of death must be submitted by the person who has the duty to submit the relevant notification within seven days from the day on which they came to know of the death (or if the death took place abroad, within three months from the day on which the person came to know of that fact).
死亡の届出は、届出義務者が、死亡の事実を知つた日から七日以内（国外で死亡があつたときは、その事実を知つた日から三箇月以内）に、これをしなければならない。
The written notification must state the following particulars and be accompanied by a medical certificate or autopsy report:
届書には、次の事項を記載し、診断書又は検案書を添付しなければならない。
the date and time and place of death; and
死亡の年月日時分及び場所
other particulars specified by Ministry of Justice Order.
その他法務省令で定める事項
If a medical certificate or autopsy report is unavailable due to unavoidable circumstances, a document proving the fact of death may be substituted for them. In this case, the grounds for the lack of availability of a medical certificate or autopsy report must be entered in the written notification.
やむを得ない事由によつて診断書又は検案書を得ることができないときは、死亡の事実を証すべき書面を以てこれに代えることができる。この場合には、届書に診断書又は検案書を得ることができない事由を記載しなければならない。
