A person who seeks to acknowledge parentage of a child must state this and the following particulars in a written notification and submit it:
認知をしようとする者は、左の事項を届書に記載して、その旨を届け出なければならない。
when the father acknowledges paternity, the name and registered domicile of the mother;
父が認知をする場合には、母の氏名及び本籍
when the person acknowledges parentage of a child who has died, the date of the child's death, as well as the name, date of birth, and registered domicile of any of the child's lineal descendants.
死亡した子を認知する場合には、死亡の年月日並びにその直系卑属の氏名、出生の年月日及び本籍
