別表第一（第三条の八関係）
(Remarks) (i) Visual acuity is to be measured using the international visual acuity chart, and for a person with a refractive error, it is to be measured by corrected visual acuity. (ii) "Loss of a finger" means loss of the thumb at the interphalangeal joint or above, or loss of any other finger at the proximal interphalangeal joint or above. (iii) "Loss of the use of a finger" means loss of half or more of the distal phalanx of the finger, or leaving a marked impairment of motion in the metacarpophalangeal joint or the proximal interphalangeal joint (for the thumb, the interphalangeal joint). (iv) "Loss of the use of a toe" means, for the first toe, loss of half or more of the distal phalanx, and for any other toe, loss at the distal interphalangeal joint or above, or leaving a marked impairment of motion in the metatarsophalangeal joint or the proximal interphalangeal joint (for the first toe, the interphalangeal joint).
（備考） 一 視力の測定は、万国式試視力表によるものとし、屈折異常があるものについては、矯正視力によつて測定する。 二 指を失つたものとは、おや指は指節間関節、その他の指は近位指節間関節以上を失つたものをいう。 三 指の用を廃したものとは、指の末節の半分以上を失い、又は中手指節関節若しくは近位指節間関節（おや指にあつては指節間関節）に著しい運動障害を残すものをいう。 四 趾の用を廃したものとは、第一趾は末節の半分以上、その他の趾は遠位趾節間関節以上を失つたもの又は中足趾節関節若しくは近位趾節間関節（第一趾にあつては趾節間関節）に著しい運動障害を残すものをいう。
Visual impairment as listed below:
次に掲げる視覚障害
Both eyes with visual acuity reduced to 0.1 or less each;
両眼の視力がそれぞれ〇・一以下に減じたもの
As a result of measurement with a Goldmann perimeter, the sum of the peripheral visual field angles of both eyes with the I/4 test object has been reduced to 80 degrees or less each;
ゴールドマン型視野計による測定の結果、両眼のⅠ／四視標による周辺視野角度の和がそれぞれ八〇度以下に減じたもの
As a result of measurement with an automated perimeter, the number of visible points with both eyes open has been reduced to 70 points or less;
自動視野計による測定の結果、両眼開放視認点数が七〇点以下に減じたもの
Hearing in both ears reduced to the extent that ordinary speech cannot be understood at a distance of 40 centimeters or more;
両耳の聴力が、四〇センチメートル以上では通常の話声を解することができない程度に減じたもの
Leaving considerable impairment of the function of mastication or speech;
そしやく又は言語の機能に相当程度の障害を残すもの
Leaving marked impairment of the function of the spinal column;
脊柱の機能に著しい障害を残すもの
Loss of the use of two of the three major joints of one upper limb;
一上肢の三大関節のうち、二関節の用を廃したもの
Loss of the use of two of the three major joints of one lower limb;
一下肢の三大関節のうち、二関節の用を廃したもの
Leaving a false joint in a long tubular bone and leaving marked impairment of motor function;
長管状骨に偽関節を残し、運動機能に著しい障害を残すもの
Loss of the thumb and the index finger of one upper limb, or loss of three or more fingers of one upper limb including the thumb or the index finger;
一上肢のおや指及びひとさし指を失つたもの又はおや指若しくはひとさし指を併せ一上肢の三指以上を失つたもの
Loss of the use of four fingers of one upper limb, including the thumb and the index finger;
おや指及びひとさし指を併せ一上肢の四指の用を廃したもの
Loss of one lower limb at the Lisfranc joint or above;
一下肢をリスフラン関節以上で失つたもの
Loss of the use of all ten toes of both lower limbs;
両下肢の十趾の用を廃したもの
In addition to those listed in the preceding items, leaving an impairment of bodily function to the extent that labor is significantly restricted or that significant restrictions need to be imposed on labor;
前各号に掲げるもののほか、身体の機能に、労働が著しい制限を受けるか、又は労働に著しい制限を加えることを必要とする程度の障害を残すもの
Leaving an impairment of the mental or nervous system to the extent that labor is significantly restricted or that significant restrictions need to be imposed on labor;
精神又は神経系統に、労働が著しい制限を受けるか、又は労働に著しい制限を加えることを必要とする程度の障害を残すもの
A condition in which an injury or illness has not healed and an impairment of bodily function, or of the mental or nervous system, remains to the extent that labor is restricted or that restrictions need to be imposed on labor, as specified by the Minister of Health, Labour and Welfare.
傷病が治らないで、身体の機能又は精神若しくは神経系統に、労働が制限を受けるか、又は労働に制限を加えることを必要とする程度の障害を有するものであつて、厚生労働大臣が定めるもの
