The mayor of a municipality (including the head of a special ward and, in the case of a designated city under Article 252-19, paragraph (1) of the Local Autonomy Act, a ward head or a general ward head) may, pursuant to the provisions of the Municipal Ordinance of the municipality, certify free of charge, to an Association of Medical Care Systems for the Elderly Aged 75 and older or a person who receives old-old medical care benefit, matters concerning the family register of the insured or a person who was insured.
市町村長（特別区の区長を含むものとし、地方自治法第二百五十二条の十九第一項の指定都市にあつては、区長又は総合区長とする。）は、後期高齢者医療広域連合又は後期高齢者医療給付を受ける者に対し、当該市町村の条例で定めるところにより、被保険者又は被保険者であつた者の戸籍に関し、無料で証明を行うことができる。
