Persons who seek to divorce must state this and the following particulars in the written notification and submit it:
離婚をしようとする者は、次に掲げる事項を届書に記載して、その旨を届け出なければならない。
the name of the party to be designated as the person with parental authority (or, if a petition for adjudication of domestic relations or conciliation of domestic relations seeking the designation of the person with parental authority has been filed, a statement to that effect), and the name of the children over whom that party is to exercise parental authority; and
親権者と定められる当事者の氏名（親権者の指定を求める家事審判又は家事調停の申立てがされている場合にあつては、その旨）及びその者が親権を行う子の氏名
