Article 21Application, etc. under Article 28, paragraph (3) of the Supplementary Provisions of the amending Act
第二十一条（改正法附則第二十八条第三項の申請等）
An application pursuant to the provisions of Article 28, paragraph (3) of the Supplementary Provisions of the amending Act must be made by submitting one written application in Appended Form No. 16 and one photograph.
改正法附則第二十八条第三項の規定による申請は、別記第十六号様式による申請書一通及び写真一葉を提出して行わなければならない。
In filing the application referred to in the preceding paragraph, the applicant must present their passport and the registration certificate deemed to be a special permanent resident certificate pursuant to the provisions of Article 28, paragraph (1) of the Supplementary Provisions of the amending Act. In this case, a special permanent resident who is unable to present their passport must submit one document stating the reason therefor.
In the case of filing the application referred to in paragraph (1) for a special permanent resident who is under 16 years of age, submission of a photograph is not required.
The document of which the mayor of the municipality makes a copy pursuant to the provisions of Article 22, paragraph (1) of the Arrangement and Transitional Measures Cabinet Order as applied mutatis mutandis pursuant to Article 23, paragraph (1) of the Arrangement and Transitional Measures Cabinet Order and sends the copy to the Commissioner of the Immigration Services Agency is the passport presented pursuant to the provisions of paragraph (2).
整備及び経過措置政令第二十三条第一項において準用する整備及び経過措置政令第二十二条第一項の規定により市町村の長が写しを作成し、当該写しを出入国在留管理庁長官に送付する書類は、第二項の規定により提示された旅券とする。
