A person who seeks to take the surname of their mother or father pursuant to the provisions of Article 791, paragraphs (1) through (3) of the Civil Code must state this and the name and registered domicile of the father or mother in a written notification and submit it.
民法第七百九十一条第一項から第三項までの規定によつて父又は母の氏を称しようとする者は、その父又は母の氏名及び本籍を届書に記載して、その旨を届け出なければならない。
