別表障害の範囲（第二条、第四十八条関係）
| (i) Permanent visual impairment as set forth below (a) Visual acuity of both eyes (measured with an international visual acuity chart, or, for a person with a refractive error, measured as corrected visual acuity; the same applies hereinafter) of 0.1 or less in each eye (b) Visual acuity of 0.02 or less in one eye and 0.6 or less in the other eye (c) Visual field of 10 degrees or less in each eye (d) A loss of one-half or more of the visual field of both eyes (ii) Permanent impairment of hearing or of balance function as set forth below (a) Hearing level of 70 decibels or more in each ear (b) Hearing level of 90 decibels or more in one ear and 50 decibels or more in the other ear (c) Best speech discrimination score for ordinary speaking voice with both ears of 50 percent or less (d) Significant impairment of balance function (iii) Impairment of voice function, language function or mastication function as set forth below (a) Loss of voice function, language function or mastication function (b) Significant and permanent impairment of voice function, language function or mastication function (iv) Physical disability of the limbs or trunk as set forth below (a) Significant and permanent impairment of the function of one upper limb, one lower limb or the trunk (b) Absence of the thumb of one upper limb at or above the interphalangeal joint, or absence of two or more fingers of one upper limb, including the index finger, each at or above the first interphalangeal joint (c) Absence of one lower limb at or above the Lisfranc joint (d) Significant and permanent impairment of the function of the thumb of one upper limb, or of the function of three or more fingers of one upper limb including the index finger (e) Absence of all toes of both lower limbs (f) Impairment, other than those set forth in (a) through (e), that is found to be of a degree equal to or greater than the degree of the impairments set forth in (a) through (e) (v) Impairment of the function of the heart, kidneys or respiratory organs, or other impairment specified by Cabinet Order, that is permanent and found to be of a degree that significantly restricts daily life |
| 一 次に掲げる視覚障害で永続するもの イ 両眼の視力（万国式試視力表によつて測つたものをいい、屈折異状がある者については、矯正視力について測つたものをいう。以下同じ。）がそれぞれ〇・一以下のもの ロ 一眼の視力が〇・〇二以下、他眼の視力が〇・六以下のもの ハ 両眼の視野がそれぞれ一〇度以内のもの ニ 両眼による視野の二分の一以上が欠けているもの 二 次に掲げる聴覚又は平衡機能の障害で永続するもの イ 両耳の聴力レベルがそれぞれ七〇デシベル以上のもの ロ 一耳の聴力レベルが九〇デシベル以上、他耳の聴力レベルが五〇デシベル以上のもの ハ 両耳による普通話声の最良の語音明瞭度が五〇パーセント以下のもの ニ 平衡機能の著しい障害 三 次に掲げる音声機能、言語機能又はそしやく機能の障害 イ 音声機能、言語機能又はそしやく機能の喪失 ロ 音声機能、言語機能又はそしやく機能の著しい障害で、永続するもの 四 次に掲げる肢体不自由 イ 一上肢、一下肢又は体幹の機能の著しい障害で永続するもの ロ 一上肢のおや指を指骨間関節以上で欠くもの又はひとさし指を含めて一上肢の二指以上をそれぞれ第一指骨間関節以上で欠くもの ハ 一下肢をリスフラン関節以上で欠くもの ニ 一上肢のおや指の機能の著しい障害又はひとさし指を含めて一上肢の三指以上の機能の著しい障害で、永続するもの ホ 両下肢のすべての指を欠くもの ヘ イからホまでに掲げるもののほか、その程度がイからホまでに掲げる障害の程度以上であると認められる障害 五 心臓、じん臓又は呼吸器の機能の障害その他政令で定める障害で、永続し、かつ、日常生活が著しい制限を受ける程度であると認められるもの |
