A petition for mediation must be filed by submitting a written petition to the court.
調停の申立ては、申立書を裁判所に提出してしなければならない。
The written petition referred to in the preceding paragraph must state the following matters:
前項の申立書には、次に掲げる事項を記載しなければならない。

当事者及び法定代理人
