When a Municipality finds it necessary in relation to an insured person's eligibility, insurance benefit or insurance premiums, it may request a public agency to provide access to necessary documents or to provide necessary materials or may request a report from a bank, trust company or any other institution or the insured person's employer or any other person involved, concerning matters relating to the acquisition and loss of eligibility as an insured person, matters necessary to confirm that the reason for which an insured person received insurance benefits arose from an act of a third party, the status of assets or income of the insured person or of the Householder of the household to which the insured person belongs, or any change in the type of insured person under the National Pension or the status of payment of insurance premiums pursuant to the provisions of the National Pension Act.
市町村は、被保険者の資格、保険給付及び保険料に関し必要があると認めるときは、被保険者の資格の取得及び喪失に関する事項、被保険者の保険給付を受けた事由が第三者の行為によつて生じたものであることを確認するために必要な事項、被保険者若しくは被保険者の属する世帯の世帯主の資産若しくは収入の状況又は国民年金の被保険者の種別の変更若しくは国民年金法の規定による保険料の納付状況につき、官公署に対し、必要な書類の閲覧若しくは資料の提供を求め、又は銀行、信託会社その他の機関若しくは被保険者の雇用主その他の関係者に報告を求めることができる。
When a Municipality finds it necessary in relation to an insured person's eligibility, it may request another Municipality or any Society or any Insurer or mutual aid association under the provisions of any of the Acts listed in items (i) through (iii) of Article 6, or the Promotion and Mutual Aid Corporation for Private Schools of Japan, which shall administer the Private School Personnel Mutual Aid System pursuant to the provisions of the Private School Personnel Mutual Aid Association Act, to provide the name and address of an insured person covered by the National Health Insurance provided by such another Municipality or Society, or an insured person covered by health insurance or seaman's insurance or said person's dependent, or a member of a mutual aid association or said member's dependent, or a subscriber to the Private School Personnel Mutual Aid System or said subscriber's dependent, the name and location of the applicable office as set forth in Article 3, paragraph (3) of the Health Insurance Act, or any other necessary materials.
市町村は、被保険者の資格に関し必要があると認めるときは、他の市町村、組合、第六条第一号から第三号までに掲げる法律の規定による保険者若しくは共済組合又は私立学校教職員共済法の規定により私立学校教職員共済制度を管掌することとされた日本私立学校振興・共済事業団に対し、他の市町村若しくは組合が行う国民健康保険の被保険者、健康保険若しくは船員保険の被保険者若しくは被扶養者、共済組合の組合員若しくは被扶養者又は私立学校教職員共済制度の加入者若しくは被扶養者の氏名及び住所、健康保険法第三条第三項に規定する適用事業所の名称及び所在地その他の必要な資料の提供を求めることができる。
