The head of a Municipality (or, in the case of a designated city as set forth in Article 252-19, paragraph (1) of the Local Autonomy Act, the head of a ward or the head of a comprehensive ward) may, pursuant to the provisions of the Municipal Ordinance of such Municipality, issue, free of charge, to a Municipality, a Society or a person who receives insurance benefits, a certificate concerning the family register of a current or former insured person.
市町村長（地方自治法第二百五十二条の十九第一項の指定都市にあつては、区長又は総合区長とする。）は、当該市町村の条例で定めるところにより、市町村若しくは組合又は保険給付を受ける者に対し、被保険者又は被保険者であつた者の戸籍に関し、無料で証明を行うことができる。
