A mayor of a Municipality must, without delay, in the following cases, notify the prefectural governor of the name of said Designated Provider of Community-Based Service for Preventive Long-Term Care, the location of the Business Office pertaining to said designation, and other matters specified by Order of the Ministry of Health, Labour and Welfare, and also provide public notice thereof:
市町村長は、次に掲げる場合には、遅滞なく、当該指定地域密着型介護予防サービス事業者の名称、当該指定に係る事業所の所在地その他の厚生労働省令で定める事項を都道府県知事に届け出るとともに、これを公示しなければならない。
when an appointment as service provider as set forth in the main clause of Article 54-2, paragraph (1) is made;
第五十四条の二第一項本文の指定をしたとき。
when notification of abolishment of business pursuant to the provisions of Article 115-15, paragraph (2) has been provided;
第百十五条の十五第二項の規定による事業の廃止の届出があったとき。
when an appointment as service provider as set forth in the main clause of Article 54-2, paragraph (1) is rescinded, or the effect of the whole or a part of an appointment as service provider is suspended, pursuant to the provisions of the preceding Article.
前条の規定により第五十四条の二第一項本文の指定を取り消し、又は指定の全部若しくは一部の効力を停止したとき。
