A certification pursuant to the provisions of Article 78 of the Act must be obtained according to Form No. 15 from the Director of the Labor Standards Office Concerned.In this case, the employer shall at same time submit a written document that proves the fact that there has been gross negligence prescribed in the provisions said Article.
法第七十八条の規定による認定は、様式第十五号により、所轄労働基準監督署長から受けなければならない。この場合においては、使用者は、同条に規定する重大な過失があつた事実を証明する書面をあわせて提出しなければならない。
