An employer who intends to receive the issuance of a starting ticket pursuant to the provisions of paragraph (1) of the preceding Article must, in advance, submit a written application stating the following matters to the Revenue Collecting Officer of the Prefectural Labor Bureau for the payment register and receive the issuance of a starting ticket receipt book (Form No. 2):
