An employer of businesses deemed to be one business pursuant to the provisions of Article 7 of the Act must, within 40 days counting from June 1 of the following insurance year or within 50 days counting from the day on which the insurance relation became extinct, submit to the Revenue Collecting Officer of the Prefectural Labor Bureau Concerned a report stating the following matters:
