Form

様式(ようしき)

A prescribed form — the notification or application someone actually has to submit. Forms are usually set out at the end of a Ministerial Order (省令), such as the Ordinance for Enforcement of the Labor Standards Act (労働基準法施行規則), and e-Gov provides them as PDF files rather than text, so this site links to the form itself on e-Gov.
