第二十条の四（労災保険率特例適用申告書）
The matters specified by Order of the Ministry of Health, Labour and Welfare under Article 12-2 of the Act are as follows:
法第十二条の二の厚生労働省令で定める事項は、次のとおりとする。
Labor insurance number;
労働保険番号
Name of the business and the place where the business is conducted;
事業の名称及び事業の行われる場所
The employer's name or title, and address or location;
事業主の氏名又は名称及び住所又は所在地
an outline of the business conducted by the employer;
事業主が行う事業の概要
the number of workers constantly employed by the employer; and
事業主が常時使用する労働者数
the measures referred to in the preceding Article that the employer has taken to secure safety or health of the workers, and the insurance year in which those measures were taken.
事業主が講じた前条の労働者の安全又は衛生を確保するための措置及び当該措置の講じられた保険年度
The matters listed in item (vi) of the preceding paragraph must be confirmed by the Director of the Prefectural Labor Bureau having jurisdiction over the location of the workplace.
前項第六号に掲げる事項については、事業場の所在地を管轄する都道府県労働局長の確認を受けなければならない。
Documents by which it can be made clear that the measures referred to in the preceding Article to secure safety or health of the workers have been taken must be attached to the written declaration under Article 12-2 of the Act.
