第百二十二条（株主名簿記載事項を記載した書面の交付等）
Article 122Delivery of Documents Showing Information Required to Be Entered in the Shareholder Register
前条第一号の株主は、株式会社に対し、当該株主についての株主名簿に記載され、若しくは記録された株主名簿記載事項を記載した書面の交付又は当該株主名簿記載事項を記録した電磁的記録の提供を請求することができる。
A shareholder as referred to in item (i) of the preceding Article may file a request with the stock company to be issued a document showing the information required to be entered in the shareholder register which has been entered or recorded in the shareholder register with respect to that shareholder, or to be provided with the electronic or magnetic record in which the information required to be entered in the shareholder register has been recorded.
前項の書面には、株式会社の代表取締役（指名委員会等設置会社にあっては、代表執行役。次項において同じ。）が署名し、又は記名押印しなければならない。
The documents referred to in the preceding paragraph must be affixed with the signature, or name and seal, of the representative director of the stock company (referring to the representative executive officer for a company with nominating committee, etc.; the same applies in the following paragraph).
With respect to an electronic or magnetic record referred to in paragraph (1), the representative director of the stock company must implement measures in lieu of the affixing of the signature, or name and seal that is prescribed by Ministry of Justice Order.
The provisions of the preceding three paragraphs do not apply to a share certificate-issuing company.
