A person who intends to receive the designation under Article 23, paragraph (3) of the Act must submit a written application stating the following matters to the Minister of Health, Labour and Welfare:
法第二十三条第三項の指定を受けようとする者は、次に掲げる事項を記載した申請書を厚生労働大臣に提出しなければならない。
The applicant's name or title and address or location;
申請者の氏名又は名称及び住所又は所在地
The name or title and address or location of the manufacturer of the stamp insurance premium payment register for which the designation is sought;
当該指定を受けようとする印紙保険料納付計器の製造者の氏名又は名称及び住所又は所在地
The name, model, structure, function and method of operation of the stamp insurance premium payment register for which the designation is sought.
当該指定を受けようとする印紙保険料納付計器の名称、型式、構造、機能及び操作の方法
A person who has submitted the written application under the preceding paragraph must present the instrument for which the designation is sought to the Minister of Health, Labour and Welfare.
前項の申請書を提出した者は、当該指定を受けようとする計器を厚生労働大臣に提示しなければならない。
The designation under Article 23, paragraph (3) of the Act is to be made by public notice of the name, model, structure and function of the instrument to be designated.
法第二十三条第三項の指定は、当該指定をしようとする計器の名称、型式、構造及び機能を告示することにより行なうものとする。
