Order for Enforcement of the Health Insurance Act

in the case where high-cost medical expenses are paid pursuant to Article 41, paragraph (1): the amount specified in each of (a) through (e)(m) in accordance with the category of persons listed in (a) through (e)(m) below:

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (i) of the preceding Article: the total sum of 85,800342,000 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 286,0001,140,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 286,0001,140,000 yen, if that amount is less than 286,0001,140,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 44,400140,100 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (ii) of the preceding Article: the total sum of 270,300303,000 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 901,0001,010,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 901,0001,010,000 yen, if that amount is less than 901,0001,010,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 140,100 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (iv) of the preceding Article: the total sum of 209,400 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 698,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 698,000 yen, if that amount is less than 698,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 93,000 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (v) of the preceding Article: the total sum of 194,400 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 648,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 648,000 yen, if that amount is less than 648,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 93,000 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (vi) of the preceding Article: the total sum of 179,100 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 597,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 597,000 yen, if that amount is less than 597,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 93,000 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (vii) of the preceding Article: the total sum of 110,400 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 368,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 368,000 yen, if that amount is less than 368,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 44,400 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (viii) of the preceding Article: the total sum of 98,100 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 327,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 327,000 yen, if that amount is less than 327,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 44,400 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (ix) of the preceding Article: the total sum of 85,800 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 286,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 286,000 yen, if that amount is less than 286,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 44,400 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (xii) of the preceding Article: 61,500 yen; provided, however, that in the case of repeated high-cost medical expenses, the amount is 34,500 yen.

a person who has received certification by the insurer, as provided by Order of the Ministry of Health, Labour and Welfare, as falling under a person listed in paragraph (1), item (v)(xiii) of the preceding Article: 36,900 yen; provided, however, that in the case of repeated high-cost medical expenses, the amount is 24,600 yen.

in the case where high-cost medical expenses are paid pursuant to Article 41, paragraph (3): the amount specified in each of (a) through (f)(n) in accordance with the category of persons listed in (a) through (f)(n) below:

a person other than those listed in (b) through (f): 61,500 yenlisted in paragraph (3), item (i) of the preceding Article: the total sum of 342,000 yen and the amount obtained by multiplying by 1% the amount obtained by deducting 1,140,000 yen from the amount of expenses required for that medical treatment, calculated for that medical treatment as provided by Order of the Ministry of Health, Labour and Welfare (or 1,140,000 yen, if that amount is less than 1,140,000 yen) (where that amount includes a fractional amount of less than 1 yen, the amount obtained by rounding down that fractional amount if it is less than 50 sen, or by rounding it up to 1 yen if it is 50 sen or more); provided, however, that in the case of repeated high-cost medical expenses, the amount is 44,400140,100 yen.

The provisions of Article 41, Article 42 (excluding limited to the parts relating to paragraph (1), items (ii) through (iv)(ix) and (xiii), paragraph (2), items (ii) through (iv)(ix) and (xiii), paragraph (3), items (ii)(xii) through (iv)(xiv), paragraph (4), items (ii) through (iv), paragraph (7), item (i), (b) (xii) through (xiv), paragraph (5), items (ii) through (iv), paragraph (6), paragraph (7), item (i), (i) and (m), item (ii), (l) through (d)(n) and item (ii)(iii), (b) through (d), paragraph (9), item (ii), and paragraphs (10) through (12)(8) and paragraph (9), item (i)) and Article 43 (excluding the parts relating to paragraph (1), item (i), (b)(a) through (d)(h) and (j) through (l), item (ii), (b)(a) through (d)(k) and item (iii), (b)(a) through (d)(k)) apply mutatis mutandis to the payment of high-cost medical expenses for specially-insured day laborers. In this case, the phrase "an insured person whose standard monthly remuneration for the month in which the medical treatment was received is 280,000 yen or more and less than 360,000 yen, or a dependent of such a person (excluding those listed in item (xiii))" in Article 42, paragraph (1), item (ix) is deemed to be replaced with "a person other than those listed in item (xiii)", and the phrase "an insured person whose standard monthly remuneration for the month in which the medical treatment was received is less than 160,000 yen, or a dependent of such a person (excluding those listed in the following item or item (xiv))" in paragraph (3), item (xii) of that Article is deemed to be replaced with "a person other than those listed in the following item and item (xiv)".

The provisions of Article 41-2, paragraph (1) (excluding the parts relating to items (ii), (iv), (viii), (x), (xiv) and (xvi)), paragraph (2) of that Article (excluding the parts relating to items (ii), (iv), (viii), (x), (xiv) and (xvi) of paragraph (1)), paragraphs (8) through (10) of that Article, Article 41-3, Article 42, paragraph (10), paragraph (11) of that Article (excluding the parts relating to items (ii) and (iii)through (iv)) and paragraph (12) of that Article (excluding the parts relating to items (ii) and (iii)through (iv)) apply mutatis mutandis to the payment of high-cost medical expenses for persons who are, on the reference date, specially-insured day laborers and persons who are dependents thereof.
