an authenticated copy, transcript or extract of a record which states an agreement reached through mediation, or states to the effect that the court has closed the case while deciding not to conduct a mediation or considering that mediation has been unsuccessful; and
調停において成立した合意を記載し、又は調停をしないものとして、若しくは調停が成立しないものとして事件が終了した旨を記載した調書の正本、謄本又は抄本
