In order to stabilize the finances of the national health insurance program provided by prefectures, etc., the national government shall, pursuant to Cabinet Order provisions, bear thirty-two hundredths of the total sum of the amounts listed in the following items for prefectures, with respect to the costs incurred by the Municipalities within each prefecture in providing benefits for medical treatment and paying expenses for Dietary Treatment for inpatients, expenses for Living Support for inpatients, medical expenses combined with treatment outside insurance coverage, medical expenses, medical expenses for home-nursing, special medical expenses, transport expenses, high-cost medical expenses and expenses for high-cost medical treatment combined with long-term care (in Article 73, paragraph (1), Article 75-2, paragraph (1), Article 76, paragraph (2) and Article 104 referred to as "cost incurred in providing benefits for medical treatment, etc.") and the costs incurred by the prefecture in paying the young-old payment under the provisions of the Act on Assurance of Medical Care for Elderly People (hereinafter referred to as "young-old payment"), the old-old aid under the provisions of that Act (hereinafter referred to as "old-old aid"), the long-term care payment, the contribution for securing medical care in the early phase of an epidemic and the children and child-rearing support levy:
国は、都道府県等が行う国民健康保険の財政の安定化を図るため、政令で定めるところにより、都道府県に対し、当該都道府県内の市町村による療養の給付並びに入院時食事療養費、入院時生活療養費、保険外併用療養費、療養費、訪問看護療養費、特別療養費、移送費、高額療養費及び高額介護合算療養費の支給に要する費用（第七十三条第一項、第七十五条の二第一項、第七十六条第二項及び第百四条において「療養の給付等に要する費用」という。）並びに当該都道府県による高齢者の医療の確保に関する法律の規定による前期高齢者納付金（以下「前期高齢者納付金」という。）及び同法の規定による後期高齢者支援金（以下「後期高齢者支援金」という。）、介護納付金、流行初期医療確保拠出金並びに子ども・子育て支援納付金の納付に要する費用について、次の各号に掲げる額の合算額の百分の三十二を負担する。

the amount of expenses incurred in paying the young-old payment, the old-old aid, the long-term care payment, the contribution for securing medical care in the early phase of an epidemic and the children and child-rearing support levy (or, where there is a young-old subsidy under the provisions of the Act on Assurance of Medical Care for Elderly People (hereinafter referred to as "young-old subsidy"), the amount calculated by deducting such young-old subsidy from such amount of expenses).
前期高齢者納付金及び後期高齢者支援金、介護納付金、流行初期医療確保拠出金並びに子ども・子育て支援納付金の納付に要する費用の額（高齢者の医療の確保に関する法律の規定による前期高齢者交付金（以下「前期高齢者交付金」という。）がある場合には、これを控除した額）
With regard to the application of the provisions of the preceding paragraph to a prefecture to which belongs a Municipality that has reduced the ratio of co-payment pursuant to the provisions of Article 43, paragraph (1), or a Municipality for which a prefecture or a Municipality has chosen to bear all or part of the amount equivalent to the co-payment for all or some of the insured persons, the amount listed in item (i) of that paragraph shall be the amount equivalent to the amount listed in that item calculated pursuant to Cabinet Order provisions as if the relevant measures to reduce the ratio of co-payment or to bear all or part of the amount equivalent to the co-payment were not taken.
In addition to what is provided for in paragraph (1), the national government shall, as provided by Cabinet Order, bear for prefectures an amount equal to one-fourth of the total amount of expenses incurred for benefits relating to medical services of an amount or more calculated, as provided by Cabinet Order, as benefits relating to medical services that have a significant impact on national health insurance finances, taking into consideration the ratio of expenses incurred for high-cost benefits relating to medical services to expenses incurred for all benefits relating to medical services for insured persons, etc. (referred to as the "amount of high-cost medical expenses subject to sharing" in Article 72-2, paragraph (2)).
国は、第一項に定めるもののほか、政令で定めるところにより、都道府県に対し、被保険者に係る全ての医療に関する給付に要する費用の額に対する高額な医療に関する給付に要する費用の割合等を勘案して、国民健康保険の財政に与える影響が著しい医療に関する給付として政令で定めるところにより算定する額以上の医療に関する給付に要する費用の合計額（第七十二条の二第二項において「高額医療費負担対象額」という。）の四分の一に相当する額を負担する。
