Supplementary Provisions, Article 11

In the written applications in Appended Forms No. 1 and No. 2 of the New Regulation, the field in which the number of the special permanent resident certificate or the number of the residence card is to be stated is, where the special permanent resident pertaining to that statement holds a registration certificate during the period specified in the items of Article 28, paragraph (2) of the Supplementary Provisions of the Amending Act, or where the mid- to long-term resident pertaining to that statement holds a registration certificate during the period specified in the items of Article 15, paragraph (2) of the Supplementary Provisions of the Amending Act, the field in which the registration number of the registration certificate held by that special permanent resident or mid- to long-term resident is to be stated.
