第十四条（改正法附則第十五条第三項の申請等）
An application under Article 15, paragraph (3) of the Supplementary Provisions of the amending Act must be made by submitting one written application in the form of Appended Form No. 3 and one photograph.
改正法附則第十五条第三項の規定による申請は、別記第三号様式による申請書一通及び写真一葉を提出して行わなければならない。
In making the application under the preceding paragraph, the applicant must present the following documents. In this case, a mid- to long-term resident who is unable to present a passport or a certificate of status of residence must submit one document stating the reason.
a registration certificate that is deemed to be a residence card pursuant to Article 15, paragraph (1) of the Supplementary Provisions of the amending Act;
in the case of a person who has been issued a permit to engage in an activity other than that permitted under the status of residence previously granted, that permit.
資格外活動許可書の交付を受けている者にあっては、当該資格外活動許可書
When an application under paragraph (1) is made for a mid- to long-term resident under 16 years of age, submission of a photograph is not required.
