Article 17Matters to Be Stated in the Written Report of Facts to the Commissioner of the Immigration Services Agency
第十七条（出入国在留管理庁長官に対する事実の申告の書面の記載事項）
The document set forth in Article 55-74, paragraph (1) of the Act must state the following matters:
法第五十五条の七十四第一項の書面には、次に掲げる事項を記載しなければならない。
the name and age of the person making the report and the name of the immigration detention center, etc.;
申告をする者の氏名及び年齢並びに入国者収容所等の名称
the facts reported;
申告に係る事実
the date on which the facts reported occurred;
申告に係る事実があった年月日
whether the director of the immigration detention center or the director of the regional immigration services bureau gave instruction, and the content of the instruction;
入国者収容所長又は地方出入国在留管理局長の教示の有無及びその内容
the date of the report.
