An application under Article 19-15-2, paragraph (2) of the Act must be made by appearing in person at the office of the municipality (including special wards, and, in a designated city prescribed in Article 252-19, paragraph (1) of the Local Autonomy Act (Act No. 67 of 1947), wards or administratively consolidated wards; the same applies hereinafter) relating to the notification prescribed in that paragraph and submitting one written application in accordance with the Appended Form No. 2 and one photograph (limited to a photograph that satisfies all of the following items):
法第十九条の十五の二第二項の規定による申請は、同項に規定する届出に係る市町村（特別区を含むものとし、地方自治法（昭和二十二年法律第六十七号）第二百五十二条の十九第一項の指定都市にあっては、区又は総合区。以下同じ。）の事務所に自ら出頭し、別記第二号様式による申請書一通及び写真（次の各号のいずれにも該当するものに限る。）一葉を提出して行わなければならない。
a photograph in which only the person themselves is photographed;
本人のみが撮影されたもの
a photograph in which the person is not wearing a hat and is facing the front;
無帽で正面を向いたもの
a photograph with no background (including shadows);
背景（影を含む。）がないもの
a photograph that is clear;
鮮明であるもの
a photograph that was taken within six months before the date of application.
申請の日前六月以内に撮影されたもの
In making the application under the preceding paragraph, the person must present the passport. In this case, a mid- to long-term resident who is unable to present the passport must submit a document stating the reason therefor.
前項の申請に当たっては、旅券を提示しなければならない。この場合において、旅券を提示することができない中長期在留者にあっては、その理由を記載した書面一通を提出しなければならない。
When the application under paragraph (1) concerns a person less than one year of age, submission of a photograph is not required, notwithstanding the provisions of that paragraph. However, this does not apply when the Commissioner of the Immigration Services Agency has determined that submission is required.
An application under paragraph (1) need not be made by the mid- to long-term resident by appearing in person, notwithstanding the provisions of paragraph (1), where the mid- to long-term resident is under 16 years of age or finds it difficult to appear in person due to illness, physical disability or any other unavoidable reason, and any of the following persons carries out, on behalf of the mid- to long-term resident, the procedures relating to the submission of the written application, etc. prescribed in that paragraph and the presentation of the passport prescribed in paragraph (2). However, this does not apply when a request under Article 19-15-2, paragraph (3) of the Act is made together with that application.
第一項の申請は、中長期在留者が十六歳に満たない場合又は病気、身体の障害その他のやむを得ない理由により出頭することが困難である場合において、次に掲げる者が当該中長期在留者に代わって同項に定める申請書等の提出及び第二項に定める旅券の提示に係る手続をするときは、第一項の規定にかかわらず、当該中長期在留者が自ら出頭してこれを行うことを要しない。ただし、当該申請に併せて法第十九条の十五の二第三項の規定による申出をする場合は、この限りでない。
a relative of the mid- to long-term resident (excluding a person under 16 years of age);
当該中長期在留者の親族（十六歳に満たない者を除く。）
the legal representative of the mid- to long-term resident;
当該中長期在留者の法定代理人
a person who has been requested by the mid- to long-term resident or by a person listed in the preceding two items (excluding a person under 16 years of age).
当該中長期在留者又は前二号に掲げる者の依頼を受けた者（十六歳に満たない者を除く。）
Where, pursuant to the provisions of the preceding paragraph, the mid- to long-term resident is not required to appear in person, a person who intends to carry out the procedures prescribed in that paragraph on behalf of the mid- to long-term resident must present to the head of the municipality prescribed in paragraph (1) materials clarifying that the case falls under that situation, or give an explanation to that effect.
