An employer must state its name on the forms and other necessary documents to be used for applications for permission, approval, recognition or designation, notifications or reports prescribed by the Act and orders based thereon, and submit them to the administrative agency.
使用者は、法及びこれに基づく命令に定める許可、認可、認定若しくは指定の申請、届出又は報告に用いるべき様式その他必要な書類に氏名を記載し、行政官庁に提出しなければならない。
