With respect to workers falling under any of the items of Article 41 of the Act and workers made to work under the provisions of paragraph (1) of Article 41-2 of the Act, items (v) and (vi) of paragraph (1) are not required to be entered.
法第四十一条各号のいずれかに該当する労働者及び法第四十一条の二第一項の規定により労働させる労働者については第一項第五号及び第六号は、これを記入することを要しない。
