**第一項**  法第三十条の四十一第一項に規定する戸籍の附票の記載等に関する事項で政令で定めるものは、次の各号に掲げる場合の区分に応じ、当該各号に定める事項とする。

The matters concerning entries, etc. in a family register supplement prescribed in Article 30-41, paragraph (1) of the Act that are specified by Cabinet Order are the matters specified in each of the following items for the category of cases set out in that item.
