**第一項**  保護の開始を申請する者は、厚生労働省令で定めるところにより、次に掲げる事項を記載した申請書を保護の実施機関に提出しなければならない。ただし、当該申請書を作成することができない特別の事情があるときは、この限りでない。

**第五項**  第三項の通知は、申請のあつた日から十四日以内にしなければならない。ただし、扶養義務者の資産及び収入の状況の調査に日時を要する場合その他特別な理由がある場合には、これを三十日まで延ばすことができる。

The notice under paragraph (3) shall be given within fourteen days from the date on which an application has been filed; provided, however, that said period may be extended to thirty days in the case where an investigation on the state of the assets and income of persons responsible for support takes time or there is any other special reason.
