      **第一項**  二以上の医療法人が新設合併（二以上の医療法人がする合併であつて、合併により消滅する医療法人の権利義務の全部を合併により設立する医療法人に承継させるものをいう。以下この目において同じ。）をする場合には、新設合併契約において、次に掲げる事項を定めなければならない。

      When two or more medical corporations conduct a consolidation-type merger (meaning a merger between two or more medical corporations in which all of the rights and obligations of the medical corporation to be dissolved as a result of the merger must be succeeded to by the medical corporation to be established as a result of the merger; hereinafter the same applies in this Division), the following information are to be stipulated in the consolidation-type merger agreement:

        **一**  新設合併により消滅する医療法人（以下この目において「新設合併消滅医療法人」という。）の名称及び主たる事務所の所在地
