### 第五条（変更事項の届出） — Notification of Changes in Matters

**第一項**  法第四条の二第二項の厚生労働省令で定める事項は、次のとおりとする。

**第二項**  法第四条の二第二項の規定による届出は、前項各号に掲げる事項に変更を生じた日の翌日から起算して十日以内に、次に掲げる事項を記載した届書を所轄労働基準監督署長又は所轄公共職業安定所長に提出することによつて行わなければならない。

The notification under Article 4-2, paragraph (2) of the Act must be made by submitting to the Director of the Labor Standards Office Concerned or the Chief of the Public Employment Security Office Concerned, within 10 days counting from the day following the day on which a change arose in a matter listed in the items of the preceding paragraph, a written notice stating the following matters:

**第三項**  所轄労働基準監督署長又は所轄公共職業安定所長は、前項の届書が提出されたときであつて、必要と認めるときには、事業主に対し、登記事項証明書その他の第一項各号に掲げる事項を確認できる書類の提出を求めることができる。

When the written notice referred to in the preceding paragraph has been submitted, if the Director of the Labor Standards Office Concerned or the Chief of the Public Employment Security Office Concerned finds it necessary, the Director or Chief may request the employer to submit a certificate of registered matters or other documents by which the matters listed in the items of paragraph (1) can be confirmed.
