### 第八十三条の四（就業促進定着手当の支給申請手続） — Procedures for Application for Payment of Employment Promotion Settlement Allowance

  **第一項**  受給資格者は、法第五十六条の三第一項第一号に該当する者のうち同一事業主の適用事業にその職業に就いた日から引き続いて六箇月間以上雇用される者であつて、第八十三条の二に規定する者に対する就業促進手当（以下「就業促進定着手当」という。）の支給を受けようとするときは、同日から起算して六箇月目に当たる日の翌日から起算して二箇月以内に、次の各号に掲げる書類及び受給資格者証を添えて（当該受給資格者が受給資格通知の交付を受けた場合にあつては、次の各号に掲げる書類の添付に併せて個人番号カードを提示して）就業促進定着手当支給申請書（様式第二十九号の二の二）を管轄公共職業安定所の長に提出しなければならない。

  When a qualified recipient intends to receive payment of employment promotion allowance for a person who, among the persons who fall under Article 56-3, paragraph (1), item (i) of the Act, is employed continuously for six months or more from the day on which the person took up the employment in the covered undertaking of the same employer and is a person prescribed in Article 83-2 (hereinafter referred to as "employment promotion settlement allowance"), the qualified recipient must, within two months counting from the day following the day that is six months counting from that day, submit to the Chief of the competent Public Employment Security Office a written application for employment promotion settlement allowance (Form No. 29-2-2), accompanied by the documents listed in the following items and by the qualified recipient's certificate (or, where the qualified recipient has been issued a notice of recipient qualification, by presenting an Individual Number Card together with attaching the documents listed in the following items).
