### 第八条（確認の請求） — Demand for Confirmation

**第一項**  法第八条の規定による被保険者となつたこと又は被保険者でなくなつたことの確認の請求は、文書又は口頭で行うものとする。

**第三項**  第一項の規定により口頭で確認の請求をしようとする者は、前項各号に掲げる事項を同項の公共職業安定所長に陳述し、証拠があるときはこれを提出しなければならない。
