### 第二十五条（証明書による失業の認定） — Recognition of Unemployment by Certificate

    **第一項**  法第十五条第四項第一号に該当する受給資格者が証明書を提出することによつて失業の認定を受けようとするときは、その理由がやんだ後における最初の失業の認定日に管轄公共職業安定所に出頭し、受給資格者証を添えて（当該受給資格者が受給資格通知の交付を受けた場合にあつては、個人番号カードを提示して）次の各号に掲げる事項を記載した医師その他診療を担当した者の証明書を提出しなければならない。

    When a qualified recipient who falls under Article 15, paragraph (4), item (i) of the Act seeks to receive recognition of unemployment by submitting a certificate, the recipient must appear at the competent Public Employment Security Office on the first day of recognition of unemployment after the reason has ceased, and submit, attaching the qualified recipient's certificate (or, where the qualified recipient has been issued a notice of recipient qualification, presenting the Individual Number Card), a certificate of a physician or other person who handled the medical treatment, stating the matters listed in the following items:
