### 第百一条の二の十二（特定一般教育訓練に係る教育訓練給付金の支給申請手続） — Procedures for Application for Payment of Educational Training Benefits for Specified General Educational Training

  **第一項**  教育訓練給付金支給対象者であつて、特定一般教育訓練に係る教育訓練給付金の支給を受けようとするもの（以下この条において「特定一般教育訓練受講予定者」という。）は、当該特定一般教育訓練を開始する日の十四日前までに、教育訓練給付金及び教育訓練支援給付金受給資格確認票（様式第三十三号の二の二）に次の各号に掲げる書類を添えて管轄公共職業安定所の長に提出しなければならない。

  An educational training benefit recipient who intends to receive payment of educational training benefits pertaining to specified general educational training (hereinafter referred to as a "prospective trainee in specified general educational training" in this Article) must, no later than 14 days before the day on which the recipient starts that specified general educational training, submit to the Chief of the competent Public Employment Security Office an educational training benefits and educational training support benefits qualification confirmation form (Form No. 33-2-2), attaching the documents set out in the following items.

  **第二項**  管轄公共職業安定所の長は、前項の規定により教育訓練給付金及び教育訓練支援給付金受給資格確認票を提出した特定一般教育訓練受講予定者が教育訓練給付金支給対象者であつて第百一条の二の七第二号に掲げる者に該当するものと認めたときは、次の各号に掲げる事項を通知しなければならない。

  **第三項**  前項の規定による通知を受けた第百一条の二の七第二号に掲げる者に該当する教育訓練給付金支給対象者は、特定一般教育訓練に係る教育訓練給付金の支給を受けようとするときは、当該教育訓練給付金の支給に係る特定一般教育訓練を修了した日の翌日から起算して一箇月以内に、教育訓練給付金（第百一条の二の七第一号及び第二号関係）支給申請書に次の各号に掲げる書類を添えて管轄公共職業安定所の長に提出しなければならない。

  **第四項**  第二項の規定による通知を受けた第百一条の二の七第三号に掲げる者に該当する教育訓練給付金支給対象者は、特定一般教育訓練に係る教育訓練給付金の支給を受けようとするときは、当該特定一般教育訓練を修了し、当該特定一般教育訓練に係る資格を取得等し、かつ、一般被保険者又は高年齢被保険者として雇用された日の翌日から起算して一箇月以内（一般被保険者又は高年齢被保険者として雇用されている者にあつては、当該特定一般教育訓練を修了し、かつ、当該特定一般教育訓練に係る資格を取得等した日の翌日から起算して一箇月以内）に、次の各号に掲げる書類を添えて教育訓練給付金（第百一条の二の七第三号関係）支給申請書（様式第三十三号の二の三）を管轄公共職業安定所の長に提出しなければならない。

  **第五項**  教育訓練給付金支給対象者は、第一項、第三項及び前項の規定にかかわらず、職業安定局長が定めるところにより、第一項第四号、第三項第五号及び前項第五号に掲げる書類のうち職業安定局長が定めるものを添えないことができる。
