一 視力の良い方の眼の視力（万国式試視力表によつて測つたものをいい、屈折異常がある者については、矯正視力について測つたものをいう。）が〇・〇七以下のもの又は視力の良い方の眼の視力が〇・〇八かつ他方の眼の視力が手動弁以下のもの
二 両耳の聴力レベルが九〇デシベル以上のもの
三 平衡機能に著しい障害を有するもの
四 そしやく機能を欠くもの
五 音声又は言語機能に著しい障害を有するもの
六 両上しのおや指及びひとさし指又は中指を欠くもの
七 両上しのおや指及びひとさし指又は中指の機能に著しい障害を有するもの
八 一上しの機能に著しい障害を有するもの
九 一上しのすべての指を欠くもの
一〇 一上しのすべての指の機能に著しい障害を有するもの
一一 両下しのすべての指を欠くもの
一二 一下しの機能に著しい障害を有するもの
一三 一下しを足関節以上で欠くもの
一四 体幹の機能に歩くことができない程度の障害を有するもの
一五 前各号に掲げるもののほか、身体の機能に、日常生活に著しい制限を加えることを必要とする程度の障害を有するもの
一六 精神又は神経系統に、日常生活に著しい制限を加えることを必要とする程度の障害を有するもの
一七 傷病がなおらないで、身体の機能又は精神若しくは神経系統に、日常生活に著しい制限を加えることを必要とする程度の障害を有するもの
(i) Persons whose visual acuity in the better eye (measured with the international visual acuity chart and, for persons with a refractive error, measured as corrected visual acuity) is 0.07 or less, or whose visual acuity in the better eye is 0.08 and whose visual acuity in the other eye is hand movements or less
(ii) Persons whose hearing level in both ears is 90 decibels or more
(iii) Persons with a significant impairment of equilibrium function
(iv) Persons who lack the function of mastication
(v) Persons with a significant impairment of voice or speech function
(vi) Persons who lack the thumb and the index finger or middle finger of both upper limbs
(vii) Persons with a significant impairment of the function of the thumb and the index finger or middle finger of both upper limbs
(viii) Persons with a significant impairment of the function of one upper limb
(ix) Persons who lack all the fingers of one upper limb
(x) Persons with a significant impairment of the function of all the fingers of one upper limb
(xi) Persons who lack all the toes of both lower limbs
(xii) Persons with a significant impairment of the function of one lower limb
(xiii) Persons who lack one lower limb at or above the ankle joint
(xiv) Persons with an impairment of trunk function to the extent that they are unable to walk
(xv) Persons, other than those listed in the preceding items, with an impairment of bodily function to the extent that significant restrictions on daily life are necessary
(xvi) Persons with an impairment of the mind or nervous system to the extent that significant restrictions on daily life are necessary
(xvii) Persons whose injury or illness has not healed and who have an impairment of bodily function, or of the mind or nervous system, to the extent that significant restrictions on daily life are necessary
