**第一項**  第十条第一項若しくは第二項若しくは第十一条第一項の規定による届出、第十条第三項の規定により返還され、若しくは第十一条第二項（第十二条第三項、第十三条第二項及び第十四条第四項において準用する場合を含む。）の規定により交付される特別永住者証明書の受領又は第十二条第一項若しくは第二項、第十三条第一項若しくは第十四条第一項若しくは第三項の規定による申請（以下この条及び第三十四条において「届出等」という。）は、居住地（第十条第一項若しくは第二項の規定による届出又は同条第三項の規定により返還される特別永住者証明書の受領にあっては、住居地）の市町村の事務所に自ら出頭して行わなければならない。

**第二項**  特別永住者が十六歳に満たないとき、第十二条第一項の規定による申請若しくは同条第三項において準用する第十一条第二項の規定により交付される特別永住者証明書の受領をする場合であってその申請若しくは受領の日が十六歳の誕生日であるとき、又は疾病その他の事由により自ら届出等をすることができないときは、当該届出等は、次の各号に掲げる者（十六歳に満たない者を除く。）であって当該特別永住者と同居するものが、当該各号の順序により、当該特別永住者に代わってしなければならない。

If the special permanent resident is under 16, if the special permanent resident is making an application under Article 12, paragraph (1) or receiving a special permanent resident certificate to be issued pursuant to the provisions of Article 11, paragraph (2) as applied mutatis mutandis in paragraph (3) of that Article, and the date of the application or receipt is the special permanent resident's 16th birthday, or if the special permanent resident is, due to illness and other reasons, unable to make the notification, etc. in person, a person who is listed in the following items (excluding those who are under 16) and who cohabits with the relevant special permanent resident must, in the order of the relevant items, make the notification, etc. on behalf of the relevant special permanent resident.

**第三項**  届出等については、前項に規定する場合のほか、同項各号に掲げる者（十六歳に満たない者を除く。）であって特別永住者と同居するものが当該特別永住者の依頼により当該特別永住者に代わってする場合その他法務省令で定める場合には、第一項の規定にかかわらず、当該特別永住者が自ら出頭してこれを行うことを要しない。
