**第一項**  法第十条第一項の規定による届出（同条第四項の規定により同条第一項の規定による届出とみなされる届出を除く。）又は同条第二項の規定による届出（同条第五項の規定により同条第二項の規定による届出とみなされる届出を除く。）は、別記第五号様式による届出書一通を提出して行わなければならない。
