**第一項**  法第十八条第二項の規定による届出は、様式第一号により、当該事業場の所在地を管轄する労働基準監督署長（以下「所轄労働基準監督署長」という。）にしなければならない。

A notification under the provisions of paragraph (2) of Article 18 of the Act must be submitted, according to Form No.1, to the director of the labor standards office which has jurisdiction over the location of the said workplace (hereinafter referred to as "the Director of the Labor Standards Office Concerned")
