**第一項**  事業主は、雇用保険印紙を購入しようとするときは、購入申込書に購入しようとする雇用保険印紙の種類別枚数、購入年月日、労働保険番号並びに事業主の氏名又は名称及び住所又は所在地を記入し、雇用保険印紙を販売する日本郵便株式会社の営業所に提出しなければならない。
